Liability Waiver

Passenger Liability Waiver for the OCBCF chartering of a bus through Fun Time Tours of Corpus Christi to travel from Corpus Christi on Friday, March 16 to Dallas, Texas that same day, and leaving Dallas the evening of Sunday, March 18 to arrive in Corpus Christi early on the morning of Monday, March 19.

The Optimist Coastal Bend Chess Federation has chartered this bus to allow chess players and their chaperones/parents for the purpose of traveling round trip to a chess tournament in Dallas. I hereby agree that I will hold harmless OCBCF from any legal claim I may have as a result of this trip. I agree that OCBCF will not be legally liable for any injuries received by myself or anyone in my custody or care, or for any damages, monetary or otherwise, which I receive on this trip.

I agree that I will be solely responsible for the supervision, care, and custody, during the entire length of this trip including through my eventual return to Corpus Christi, of the following passengers who are under age 18:

I agree that I and everyone in my care and custody will abide by the following rules:

1. No smoking or drinking on the bus.
2. No horseplay, name calling, rude or obnoxious behavior on the bus.
3. No inappropriate physical behavior on the bus.
4. I agree that designated OCBCF office or representative will have complete and sole authority to make any decisions regarding alterations to the scheduled bus stops, as well as any decision regarding whether a passenger will be allowed to continue on the bus trip in the event of inappropriate behavior.

In the event of inappropriate behavior the designated Optimist Coastal Bend Chess Federation officer or designee has authority to require the offending passenger to disembark at the nearest town or city. Further, I agree to be held liable for any damages to the bus, other passengers or their possessions that may result from my actions.

Instructions: Each passenger on the bus must provide a signed copy of the waiver form before boarding the bus on Friday, March 16. Passengers of less than 18 years old must have their waiver signed by their parent or guardian.
